Ads showing in excluded areas?

I have carefully excluded people from seeing my ads in all geographic territories other than the ONE I want to advertise in. I regularly see info that our ads have been displayed in excluded countries and locations.
For example, below is a screenshot of an IP address I've tracked clicking my ad from London. I checked and London is definitely excluded. Is anyone else seeing this in their accounts? Google tell me there is no way my ads are showing in excluded areas.

1. Check camp settings (there are few options like "people in target area or interested in them" etc, choose strict one
2. Every company has its own geodatabase, just because some public base says that ip is from london doesn't mean it is that way for Google

Yes it's set to people in the target area, not interested in it. I think you probably have the answer with your second response. Thank you.

People in target area OR often in target area - doesn't have to be physical, so this is one way for it to flop sometimes. Although, if your exclusions include London, then it should be watertight.
